Your First Visit

Welcome, and thank you for choosing National Spine & Pain Centers!

We understand that you have probably seen a number of other physicians and specialists before coming to us. So, we want to make your healthcare experience as smooth as possible.

Upon scheduling your first visit with us, our representatives will inform you to bring certain items to your first visit. These include:

  • Picture ID
  • Insurance card
  • Referral from your primary care physician, if applicable
  • New Patient Forms: In order to reduce wait time at your first appointment, we ask that you have these forms completed prior to coming to your first appointment. We will email you these forms or you may click to download here: New Patient Forms
  • If you have any imaging results from an X-ray or MRI, please bring in a disk, the films, and/or the written results report
  • If coming in for a procedure, you will need to have a driver that can take you home. We will also provide you additional pre-procedure instructions.

Even if you are currently on medication for your pain condition, there is no guarantee the physician will keep you on that medication. The physician will perform a complete assessment to determine your best treatment option.
