The staff of Central Florida Pain Management look forward to welcoming you to our practice and finding solutions for your chronic pain. Our patient info helps new patients in central Florida, including the cities of Ocala and The Villages, know what to expect and how to prepare for their first appointments.
Our highest priority is to ensure that you receive the appropriate treatment in a timely manner, so this first appointment is important to determine an accurate diagnosis.
We may be able to perform the recommended treatments during the consultation appointment. In most cases, though, new patients prefer to schedule their pain services for another day that is more convenient for them. This gives them time to look over the treatment plan and associated costs.
We work with popular medical insurance carriers in the central Florida area, and many of the carriers will cover a portion of the cost of treatment. You may have out-of-pocket costs, such as co-payments, deductibles, or co-insurance. These amounts are due at the time of service, and we accept payment by cash, check, or credit card. If the procedure is not covered by your insurance or if we are not in network for your carrier, you may be considered a self-pay patient and responsible for the entire cost of your procedures. We will discuss this prior to performing the therapies in your treatment plan.
Review the information below about how to prepare for the appointment. You can also download our patient forms and have them filled out before you arrive. If you have any questions about our practice, services, or how to best prepare, please contact us at your convenience.
After you read through our patient info about your consultation and appointments at our practice, contact us to schedule a consultation with Melissa Alvarez-Perez, M.D., to schedule a time that is convenient for you. We offer services for central Florida residents, including those who live in Ocala and The Villages, at our two convenient locations.
